You start as a healthy person, which is the most mobile state: you can walk or run across the map while you search for a pickup. Find a disinfectant sprayer and you become a doctor; pick up a bat and you become a killer. Each item decides your goal for as long as you hold it.
An infected player coughs and pukes to spread the virus to other people, but cannot run. Every action an infected player takes costs health, so toilet paper scattered around the map matters — collecting it puts health back. An infected player can also look for a doctor to be cured.
A doctor's job is to spray infected players and cure as many of them as possible. A killer takes the other approach and beats infected players with the bat. Left click attacks, right click gives you a burst of speed, so chases and escapes both come down to timing your speed-up.

- Toilet paper is health: grab it whenever you pass it, especially while infected, since coughing and puking drain you.
- As a healthy person, running is your advantage — infected players cannot run, so keep your distance until you find a sprayer or bat.
