Each visit opens with a short illustrated story panel explaining why the princess has come to the clinic — one storyboard shown in the game describes Princess Scarlet, who only ever ate soft processed food, then joined a village feast and paid for it the next day.
The treatment screen then fills with a close-up of the patient's open mouth under the clinic's swing-arm lamp. A vertical tool tray sits along the left edge, and you choose an instrument from it before working on the teeth. Arrows appear over the mouth to point out where to aim, so you can see which teeth still need attention: the ones coated in grime, the discoloured ones and the damaged spots. Click and drag, or tap and slide, over those areas to apply the selected tool, then switch to another instrument for the next stage of the treatment. Keep going until the princess's smile is clean and repaired, then move on to the next patient and their own reason for visiting.
